Taylor Swift Premieres 'Patient Zero' Video at VMAs, a Gothic Ghost Story Starring Dakota Johnson and Colin Farrell

Taylor Swift turned the 2026 MTV Video Music Awards into a premiere screening for her latest directorial work. On Sunday night at the Peacock Theater in Los Angeles, Swift debuted the music video for “Patient Zero,” a gothic thriller starring Dakota Johnson and Colin Farrell, immediately after accepting the inaugural Artist Director Honor from Johnson onstage.

The video reworks the song's premise, which Swift has said was inspired by a friend receiving a phone call from an ex's new girlfriend, into a ghost story. Johnson plays Farrell's current partner, trapped in a relationship that mirrors the one Swift's character endured before her death. The Hollywood Reporter described the plot: after an incident at an event, Farrell tells Johnson he thought she was “different,” prompting Johnson to ask who she is different from. The video then cuts to Swift living through the same mistreatment, revealing that Farrell poisoned her.

The visual ends where it began, with Johnson standing at a grave that is now revealed to be Swift's. The headstone reads “Beloved wife and daughter, gone too soon,” according to E! News, and Johnson leaves a rose atop it. Cara Delevingne also appears in a scene set in a golden ballroom.

Swift described the video's influences in a social media post quoted by Rolling Stone: “I got to explore the cinematic and literary genres I love so much. I wanted to incorporate elements of gothic ghost stories, true crime, psychological thrillers all in a setting I hadn't really shot in much.” She added, “The house was a metaphor for the coldness of a relationship that's faded and atrophied.”

The premiere came two days after Swift released “The Life of a Showgirl: Encore Edition,” a deluxe version of her latest album that adds four new songs: “Patient Zero,” “Babylon,” “Pink Clouding,” and “Cleveland!” The Hollywood Reporter noted that Swift wrote on Instagram that the songs were “born out of pure gratitude for your exuberance for and celebration of the album we made.”

Swift arrived at the ceremony in a black dress with a mesh cutout and a glittered silver long skirt, according to The Hollywood Reporter. E! News reported that she wore a form-fitting black halter bodice with a silver skirt and dangling Leviev diamond earrings, and that she displayed her engagement ring and wedding band from her July marriage to Travis Kelce, who was absent due to his NFL schedule.

The video is now available to stream on Spotify and Apple Music, according to Pop Base. Swift also won Video of the Year at the ceremony for “The Fate of Ophelia,” The Hollywood Reporter reported.
